    Wirbelkörperfrakturen und Osteopenie: Augmentieren oder langstreckig? Vertebral body fractures and osteopenia: augmentation or lengthening?
    Background: Vertebral fractures are the most common manifestation of osteoporosis. If an interventional therapy is needed besides kyphoplasty, stabilisation with pedicle screws is commonly accepted. Loosening of pedicle screws is one of the major complications in posterior spinal stabilisation, especially in the osteoporotic spine. Augmentation of pedicle screws with cement or lengthening of the instrumentation are widely used to improve stability in these patients. Materials and methods: A biomechanical test was performed. Twelve osteoporotic human spines (Th11–L3) were randomised by bone mineral density into two groups. A short instrumentation was compared to an extended instrumentation using a fatigue testing setup with a cranial–caudal sinusoidal, cyclic (1.0 Hz) load with stepwise increasing peak force. Testing was performed for augmented screws as well as for nonaugmented screws. Conclusion: The stabilising effect of cement augmentation in pedicle screws might not be as beneficial as expected from biomechanical pullout tests. Lengthening of the dorsal instrumentation results in a much higher increase of stability during fatigue testing in the osteoporotic spine compared to cement augmentation.

    Intelligente Implantate für das Monitoring der Knochenheilung
    Background: Monitoring of fracture healing is a complex problem. Radiographs solely enable the estimation of calcification. Telemetric determination of bone healing: A telemetric system to mechanically assess bone healing was developed. The system consists of an electronic unit applied to a locked plate, an external reader and a ground sensor to measure externally applied loads. By correlating the externally applied load and deformation in the implant, the elasticity of the osteosynthesis is calculated. Elasticity decreases with ongoing consolidation of a fracture or nonunion and is appropriate to monitor the course of bone healing. Clinical application: Clinical application was performed in 56 patients suffering nonunions or a fracture of the femur; in 47 cases treatment was completed. In 46 of 47 patients, consolidation was achieved. Different types of healing curves could be differentiated (fast healing, slow healing, plateau with following fast healing, nonhealing). Conclusion: The system proved to be valuable for the judgement of bone healing in difficult healing situations. Routine cost-effective manufacturing is possible, because the used electronic components are derived from mass products.

    Präklinische Testung - klinische Anwendung - Schadensanalyse
    Preclinical testing - clinical application: Total hip arthroplasty has become one of the most successful procedures in orthopedics. This has been made possible through continuous improvement of the implant systems and materials accompanied by an improvement in preclinical testing and the legal framework. A further improvement in results can only be achieved if the two other decisive factors for the success of an operation are also included: intensive education and training of the surgeons combined with education and awareness of the patients. Risk assessment: The introduction of new concepts always bears the risk of new unforeseen failure mechanisms, which cannot always be prevented by established preclinical testing. This requires the careful introduction of new concepts into clinical application, combined with a standardized failure analysis, in order to reduce the number of problem cases in the worst case situation.

    Rückenmarkadaptation : bioresorbierbares Mikrosystem, Medikamentenapplikation, Stammzellen
    Background. The idea behind the research project on neuroregeneration after spinal cord injury currently funded by the Deutsche Gesetzliche Unfallversicherung (DGUV) is to combine known drug and cell therapeutic approaches with a surgical procedure. Technique. Damaged spinal cord tissue is resected followed by a tension-free adaptation of the uninjured tissue. For this purpose, a connector element (mMS, mechanical micro-connector system) was developed based on microsystem technologies. The mMS additionally allows the administration of drugs via microchannels Animal trials. In a key experiment in rats, the fixation of the spinal cord tissue, regenerative axon growth through the mMS, and improvement of locomotor function was shown. Currently, further therapeutic combination strategies include the use of substances to reduce fibrotic scar formation, to digest proteoglycans that inhibit axonal growth, stem cells to stimulate axonal growth as well as the chemokine SDF1. Discussion. In order to enable a tension-free adaptation of healthy tissue, depending on the extent of the damage, a shortening of the corresponding spine segment may be necessary. This can be achieved by total vertebral resection. It is known from the literature that total removal of spine segments is technically possible. However, shortening the spine in the course of the treatment of spinal cord damage is new. By combining cellular pharmaceutical treatments with surgical mechanical procedures the neuroregenerative therapy of spinal cord injury seems possible.

    Interprosthetic fractures of the femur
    The treatment of interprosthetic fractures is challenging and the incidence is increasing as a result of increasing patient longevity and rising arthroplasty rates. Interprosthetic fractures occur between arthroplasty stems or any intramedullary implant that has been implanted for fracture fixation. Management of these fractures is a complex undertaking. The majority of fractures are localized at the femoral supracondylar level after knee resurfacing arthroplasty. The fixation status of the arthroplasty component determines the management strategies. As a result of clinical and biomechanical studies, loosening of the implant usually requires revision arthroplasty, whereas minimally invasive reduction and locked internal fixation is the treatment of choice for stable implants. Minimally invasive surgery is necessary to not further compromise local perfusion. As locked plating provides superior biomechanical stability, the use of cerclage cables should be carefully evaluated. Complex fracture configurations, inferior bone stock or an intramedullary canal that is already occupied by support structures might obviate the implantation of long revision stems and might require a more extensive approach (e.g., total femoral arthroplasty).

    The treatment of subcutaneous Achilles tendon rupture aims toward a strong, healed tendon that allows physical and athletic activities at pretrauma levels. This stability can be attained with a minimum of complications using open or minimally invasive suturing techniques, if certain technical recommendations (early operation, no use of tourniquets, simple suture techniques, degradable suture materials, early onset of physical therapy) and contraindications (diabetes mellitus, general healing problems and local risks, restriction of venous or arterial perfusion, smoking, primarily inactive patients) are kept in mind. Operative techniques require no special infrastructure. Our experimental work on human tendons demonstrates the high primary stability of applied suture techniques that allow early physical therapy. Using this concept of operative treatment and early physical therapy with an orthosis, the therapeutic goal can be reached in the shortest time and with the lowest possible rates of rerupture and functional disability.

    Mechanische Testungen von Implantaten für die „Ventrale interkorporelle Spondylodese“ der unteren Halswirbelsäule

    Die ventrale interkorporelle Spondylodese hat sich in den letzten Jahren zum Standardverfahren bei Verletzungen an der unteren Halswirbelsäule entwickelt. Neben verschiedenen neueren winkelstabilen Implantaten finden auch noch die klassischen, nicht winkelstabilen Implantate ihre Anwendung. Mehrere biomechanische Studien haben gezeigt, daß bei Testungen von Implantatsystemen an Tier- oder Humanpräparaten die Ergebnisse durch biologische Faktoren teilweise stark beeinflußt werden. Ziel dieser Studie war es, eine Meßmethode zu entwickeln, welche eine standardisierte Testung von Implantaten unter gleichen Bedingungen ermöglicht. Die entwickelte Meßmethode wurde dann für den Vergleich von 4 verschiedenen Implantatsystemen, von denen 3 winkelstabil waren, eingesetzt. Alle Implantate wurden mittels zweier Materialprüfmaschinen (MTS-Bionix 858, UTS-002.10) in den Bewegungsrichtungen Flexion/Extension sowie Rotation untersucht. Erwartungsgemäß war die Steifigkeit der winkelstabilen Systeme signifikant höher als die der nicht winkelstabilen Systeme. Die verschiedenen winkelstabilen Systeme zeigten im Vergleich untereinander ebenfalls deutliche Unterschiede hinsichtlich der Steifigkeit. Ob die gemessenen Unterschiede zu relevanten Unterschieden bei der klinischen Anwendung führen, muß durch gezielte Studien gezeigt werden.

    Implantatverankerung in osteoporotischem Knochen mittels monokortikaler Schrauben
    (Springer, 1999) ;
    Lill, C.
    ;
    ;
    Mit zunehmendem Alter und Osteoporose ist ein Abfall der kortikalen Knochendicke zu beobachten. Viele Fixationsmethoden mit guten Ergebnissen in gesundem Knochen basieren auf einer Verankerung in der Kortikalis mittels Schrauben. Aus der Klinik werden jedoch Probleme wie Schraubenausrisse oder eine unzureichende Primärfestigkeit berichtet, wenn diese Implantate in osteoporotischem Knochen eingesetzt werden. Das Ausdünnen der Kortikalis ist dabei eine der Hauptursachen für das Versagen, aber auch das Design des Implantats bestimmt, wie physiologische Lasten durch die Schnittstelle von Schraube und Knochen übertragen werden. Ziel der Studie war, den Einfluß der kortikalen Dicke sowie der Lastrichtung am Schraubenkopf auf die maximale Haltekraft einer monokortikal verankerten Schraube zu bestimmen. Es wurden Auszugstests von Schrauben aus kortikalen Rinderknochenplättchen unterschiedlicher Dicke (1–3 mm) durchgeführt. Der Schraubenkopf war dabei wie bei einem internen Fixateur winkelstabil verankert. Der Lastwinkel wurde zwischen einer reinen Auszugsrichtung und einer Scherbelastung senkrecht zur Schraubenachse variiert. Mit Abnahme der Kortikalisdicke konnte ein signifikanter, linearer Zusammenhang zwischen der dadurch reduzierten Anzahl verankerter Gewindegänge im Knochen und der Haltekraft der Schraube beobachtet werden. Des weiteren zeigten die Ergebnisse, daß eine Veränderung der Lastrichtung von einer Auszugs- zu einer Scherbelastung der Schraube zu einer signifikanten Erhöhung der Verankerungsfestigkeit führt.

    Wirkprinzipien der winkelstabilen Platten-Schrauben-Verbindung bei Fixateur-interne-Osteosynthesen
    (Springer, 1999) ; ;

    Im Rahmen von Demonstrationsversuchen wurden typische Osteosynthesekonstruktionen unter Verwendung von Titanfixateur-interne-Implantaten an Kunststoffprobekörpern montiert und mechanisch belastet. Verglichen wurden eine herkömmliche mit einer ¶winkelstabilen Schraubenfixation (Fixateur interne) an der Platte. Bei Simulation einer Osteoporose oder limitierter Platten-Knochen-Kontaktfläche am Röhrenknochen und bei Simulation eines kurzen meta-/epiphysären Fragments mit frakturparallelen Schrauben erhöhte sich die Festigkeit deutlich durch ein winkelstabiles Schrauben-Platten-Interface. Bei einer Anordnung von 3 Schrauben in einem simulierten Femurkondylus wurden hohe Momente sowohl im Schrauben-Platten-Interface als auch durch die räumliche Abstützung der fixierten Schrauben im spongiösen Knochen übertragen.

    Mechanische Überprüfung der Winkelstabilität von internen Fixateuren und winkelstabilen Plattensystemen
    (Springer, 1999) ;
    Wie im Bereich der Wirbelsäulenimplantate schon länger etabliert wurden in den letzten Jahren auch winkelstabile Plattensysteme für den Bereich der Osteosynthese entwickelt, welche zudem eine monokortikale Schraubenverankerung erlauben. Zur Realisierung der winkelstabilen Verbindung auf kleinstem Raum (Beibehaltung der flachen Plattengeometrie) wurden die verwendeten Materialien konstruktiv ausgereizt. Ziel dieser Studie war es, zu prüfen, ob die Plattenimplantate durch die Einführung der winkelstabilen Schrauben-Platten-Verbindung mechanisch geschwächt werden. Die Testmethode orientiert sich an den Standards zur Prüfung von Fixateuren für die Wirbelsäule. Die beiden exemplarisch untersuchten Plattenfixateurtypen wurden in einer Schwenkhalterung auf einer hydraulischen Materialprüfmaschine MTS-Bionix 858.2 fixiert. Bei quasistatischer und dynamischer Belastung wurden die mechanischen Kennwerte Steifigkeit, Biegefestigkeit und Dauerschwingfestigkeit ermittelt. Beide winkelstabilen Systeme zeigten ähnliche Dauerfestigkeiten. Keines der beiden Systeme zeigte ein Versagen oder eine Lockerung der winkelstabilen Verbindung. Es traten ausschließlich plastische und elastische Verformungen an der Platte und den Schraubenschäften auf oder die Schrauben versagten durch Bruch. Diese einfache Untersuchung zeigt, daß die neuen winkelstabilen Kopplungen, wenn sie entsprechend den Herstellervorgaben appliziert werden, keinesfalls Schwachstellen an den Implantaten darstellen.
